The directions are to mix the powder with 1.8 mL diluent, which yields 250 mg/mL. How much prepared solution should the nurse administer? Record your answer using one decimal place.

A primary care provider prescribes cefazolin (Kefzol) 125 mg IM for a client. The vial contains 0.5 gm of Kefzol in powdered form. The instructions indicate to add 2 mL of sterile water to provide a solution that contains 225 mg per mL. Calculate how much volume of this medication the nurse should administer?

Solution to Question No. 1
Follow the steps -

Step 1 - Find DO - 375 mg
-
Step 2 - Find TA - Its given that - "Mix the powder in 1.8 mL diluent" so, the TA - 1.8 mL
-
Step 3 - Find DA - It is clearly written that "it yields 250 mg/mL" - It means in 1 mL the dose is 250 mg then in 1.8 mL the dose would be 250 x 1.8 mg.

Step 4 - Watch units : All units are proper.
Put the values in the formula and solve it.
The Correct Answer would be - 1.5 mL
From this question we learnt a few points -

All the information given in the question may NOT be useful to solve the question (as in this question "500 mg of powder in a vial" is of no use to solve the question)

We should always follow the steps so that we get the right answer.

At times, exact figures are NOT given in the question. We need to extract the figure we need. ( as in this question we need to extract the actual values by analyzing the words of the questions - "which yields 250 mg/mL" )

The Correct Answer is : Option no. 3

Option no. 1 is wrong as : A palatable suspension is available. It is a better alternative than opening the capsule. This option may be right if option no. 3 was NOT there.

Option no. 2 is wrong as : The route of administration CAN NOT be altered without doctor's approval.

Option no. 4 is wrong as : IM injections should be avoided because of risks for tissue injury and infection. You just need to cross check whether the calculation in Option no. 3 is correct or NOT.
This is a simple question. Solve it by applying the formula.
Solution to Question no. 3
Follow the steps -

Step 1 - Find DO - 60 mg

Step 2 - Find TA - Its given - 5 mL

Step 3 - Find DA - 125 mg (written clearly)

Step 4 - Watch units : All units are proper.
Put the values in the formula and solve it.
The Correct Answer would be - 2.4 mL
Solution to Question no. 4
This question is a bit tricky (as too much information is given in the question). You just need to take out the information we require.
We just need to follow the steps -

Step 1 : Find out DO : It is clear - 125 mg

Step 2 : Find TA : If you read the question carefully it is clearly mentioned that "Add 2 mL of sterile water". So, the TA would be - 2 mL
-
Step 3 : Find DA - If you read the question carefully it is clearly mentioned that "Solution contains 225 mg per mL" . So in 2 mL (our TA is 2 mL) the dose would be 225 x 2 = 450 mg
-
Step 4 : Watch the Units : All units are proper - in mg and in mL.
Now put the values into the formula and apply the formula
The Correct Answer would be : 0.55 mL
From this question we learnt a few points -
1. All the information given in the question may NOT be useful to solve the question (as in this question "The vial contains 0.5 gms of Kefzol" is of no use to solve the question)
2. We should always follow the steps so that we get the right answer.
3. At times, exact figures are NOT given in the question. We need to extract the figure we need. ( as in this question we need to extract the actual values by analyzing the words of the questions - "Solution contains 225 mg per mL" )
